A new client is defined as a person that vaccinates their pet(s) elsewhere. Pfister Animal Hospital requires the documents listed below be completed in full to assure that there is no confusion leading up to your petʼs stay. No reservations are held until the entire set of requirements have been fulfilled and a deposit collected. There can be no exceptions.

Required Boarding Deposit (we will contact you using the information provided to get payment information):

Deposit refunds can only be given with cancellation notice 14 days from the first day of boarding or 30 days before the first day of boarding during specific holidays (New Years, Spring Break, Memorial Day, 4th of July, Labor Day, Thanksgiving, Christmas). If cancellation notice is made after that date, the deposit will be forfeited.

*In the event of illness, I authorize Pfister Animal Hospital to maintain and hospitalize the boarding pet(s) until communication can be established.

Pfister Animal Hospital is not responsible for the loss or condition of any luxury items (bedding, toys, etc.).

To prevent the spread of infectious diseases and parasites, boarded animals must be current on all vaccines (listed below) and free of internal and external parasites (fleas, lice, etc.). Distemper/Parvo vaccine can only be honored if given within the last 12 months.

DOGS: Rabies, Distemper, Parvo, Corona, Bordetella, Fecal Exam - with negative result

CATS: Rabies, Feline Distemper, Fecal Exam - with negative result
